Change your Name, Address, Email or State of Residence

If you have moved, but still reside in the same state, you can submit the address change electronically using one of the following services:

ADDRESS CHANGES

SIRCON has an electronic service on Compliance Express. This service will allow you to electronically complete and submit address changes to the Department. There is a $2.25 charge for this service and allows you to provide timely notification to the Department.

NIPR has an electronic service called Address Change Request (ACR). This service will allow you to electronically complete and submit address changes on-line and update one, multiple, or all states with a single entry. There are no fees charged for this service and allows you to provide timely notification to the Department.

UPDATE EMAIL ADDRESS

SIRCON has an electronic service on Compliance Express. This service will allow you to electronically update your email address. There is no charge to update your email address using this service, and allows you to provide timely notification to the Department so that we can communicate with you electronically.

MOVING FROM ONE STATE TO ANOTHER

A Resident licensee who has moved from Vermont to another state must submit a request to cancel their resident license in Vermont.  If they wish to become licensed in their new resident state, they also need to request that our office issue a clearance letter. Please provide the mailing address for the letter of clearance, making sure that the request is signed and dated by the licensee. These requests can be submitted using our Fax number at (802) 828-1633.

A Non-Resident licensee who has moved from one state to another state or a Resident licensee who moved from this state to another state shall file a change of address and provide certification from the new resident state within 30 days of the change of legal residence. These changes cannot be done electronically and you must submit the address change form.  In ADDITION to the form, please enclose a Home State Certification Letter from your new state of residence, (only required if your new home state does not report licensing information into the Producer Data Base (PDB). NAME CHANGES

If you need to change your name with the Department, please submit it in writing along with a copy of the documentation that supports the name change. You can fax those documents to (802) 828-1633.
